As a client of AllCare Living Services, Inc., your home care services will be provided by staff selected through our exclusive Caregiver Competency Procedure.

Services will be provided by the coordinated plan of care along with each individual client’s request.  The following information is important for you to understand as a consumer of our services and will be explained to you by an AllCare Living Services, Inc. representative.  Please ask any questions you may have about these matters, because your signature on the service contact form indicates your understanding and agreement.  
 
 

A CLIENT HAS THE RIGHT TO:
 

1.      Be fully informed in advance about the care to be provided, including the disciplines that will furnish the care and the proposed schedule of visits.

2.     
Be fully informed in advance of any changes in the care or schedule to be provided.

3.     
Participate in planning care or changes in care.  The client may refuse all or part of his / her care to the extent permitted by law after being informed of the expected consequences of such action.

4.     
Confidentiality of clients records in accordance with Federal and State laws.  Our policies and procedures regarding disclosure of client records is that the client’s written consent is required for release of medical or financial information not authorized by law.  Client records will be protected from unauthorized disclosure or use.

5.     
Voice grievances regarding care that is or fails to be provided, without coercion, discrimination, reprisal, or unreasonable interruption of services for doing so.

6.     
Have one’s property treated with respect and voice grievances regarding lack of respect for property by anyone providing services on our behalf, and not be subjected to discrimination and reprisal for doing so.

7.     
Have AllCare Living Services, Inc. investigate complaints regarding care that is or fails to be provided, or regarding the lack of respect for the client’s property by anyone providing services on our behalf and document both the existence and resolution of such complaints.

8.     
Be fully informed orally and in writing in advance of the following:

  •  All items and services provided by or under arrangements with the agency for which payment may be expected under Medicaid or any other Federal program of which the agency is reasonably aware.
  •  Any charges the individual may have to pay regarding items and services provided by or under arrangements with the agency.
  •  Any changes in the coverage of or the charges for items and services for which the individual may be liable as soon as possible but no later than 30 calendar days from the date the agency becomes aware of such change.
9.     Have the client’s family or guardian exercise all of the client’s rights if the client is incompetent.

Be informed that AllCare Living Services, Inc. can be reached 24 hours a day by calling 843-832-9888.  If an emergency situation arises, client may contact the on-call supervisor by calling 843-832-9888.  Although not required, we request that if you have any complaints or concerns, you first attempt to resolve these directly with us.

11.  Be informed of the availability of the State Home Health Agency Hotline.  The purpose of the hotline is to answer questions, accept complaints about home health agencies and complaints concerning the implementation of advanced directive requirements.  The state hotline number is 1-800-922-6735.  Its hours of operation are 24hrs/day-7days/week.

12. 
Have the home care staff communicate in a language or form the client can reasonably be expected to understand.  Whenever possible, the home care organization will provide or assist in obtaining special devices, interpreters or other aids to facilitate communication.

13.  Be fully informed at admission of home care organization ownership and control as well as any relationship the organization has that it may bring to the organization when it makes referrals.

14.  Be told on request if the agency’s liability insurance will cover injuries to its employees when they are in the client’s home, and if the agency’s insurance will cover theft of property.
